Once your appointment is booked, you will receive an email from Chronic Care of Richmond that will look like this.

Click on "View Forms."

Click on "Register"

Enter your first name, last name, and email address.

An email will be sent to that email address.

Then return to your email inbox and click on an email from AdvancedMD.

Click on "Create Your Account."

Enter your choice of password, enter the same password again, and enter your date of birth.

Check the box to agree to the terms and conditions.

You will automatically be directed to your private Patient Portal.

Click on the Start button next to each of the forms listed to complete.

Once your forms are completed, click on “Profile” to view appointments, change profile settings, make payments, and receive messages from our team.
